PLEASE READ THE FOLLOWING CAREFULLY BEFORE SUBMITTING YOUR APPLICATION
By participating in the Midtown Tree Grant Program, you understand that you are responsible for sourcing the above mentioned trees, and that you have access to Tree OKC (OKC Beautiful) for assistance with this process. You also understand and agree to the planting schedule you have outlined above, and assume full responsibility for the care, watering, and maintenance of your trees.
You further understand that after being planted, should the tree need to be removed for any reason, you are solely responsible for said removal and proper disposal. You agree that Midtown OKC shall not be liable for any damages of any kind whatsoever arising out of or by reason of the planting of any tree or the participation in the Midtown Tree Grant Program.
By participating in the Midtown Tree Grant Program, you understand that you are accepting a two-year replacement warranty. Meaning, if your tree dies before the two-year mark, you are fully responsible for replacing said tree.
